Student Survey

Teachers should begin and end the lesson with the student survey. Please ask these two questions a few days prior to giving the lesson, and then again after the lesson is completed.

This letter includes the survey as well as facts about the importance of sun safety for children. It should be sent to parents either by email or as a hard copy a few days prior to the teacher’s presentation of the program to their students.
